Small studies suggest that oral supplementation with SAMe is not well absorbed by the body. Clinicians recommend taking oral SAMe with vitamin B12, folic acid, methionine, and trimethylglycine to enhance absorption.

S-adenosyl-L-methionine (commonly called "SAM-e") is a naturally-occurring chemical component present in all cells of the body where it is essential in more than 200 metabolic pathways. SAM-e has been approved as a prescription drug for depression in Germany, Italy, Spain, and Russia, and has been in use in Europe for over three decades. It is available without a prescription in the United States and some other countries.

When doctors found that conditions like depression and liver disease were associated with abnormal levels of S-adenosylmethionine in the body, researchers began to investigate the efficacy of SAM-e in the treatment of these types of health concerns. Overall, there has been evidence that SAM-e helps improve a host of conditions, including depression, liver disease, osteoarthritis and fibromyalgia. However, researchers agree that more studies are needed to fully understand the mechanisms and medicinal potential of the supplement. (4)

Recommended Dosage: SAMe should be taken on an empty stomach for maximum absorption. SAMe supplementation can be divided over the course of the day, (i.e. before breakfast, before lunch and before supper.) If the desired benefits are not achieved, you may increase your level of supplementation by taking 2 tablets daily (800mg) in the second week. While generally well-tolerated, the therapeutic value of SAM-e in chronic liver disease is limited (26) or yet to be determined. Because it is poorly absorbed, enteric-coated tablets are preferred. Outside the United States, parenteral formulations are used to treat fibromyalgia (9), osteoarthritis, and tendonitis as well as depression. However, patients with bipolar disorder or taking serotonergic medications including antidepressants should discuss any use of this supplement with their treating physician, as cases of mania and serotonin syndrome have been reported.

Patients on antidepressants or with bipolar disorder should avoid this supplement without the supervision of the treating physician due to case reports of mania, serotonin syndrome, or other side effects (13) (22) (46).
